Hypogammglobulnemia

Overview

Hypogammaglobulinemia is a type of primary immune deficiency disease characterized by a reduction in all types of gamma globulins. Hypogammaglobulinemia is a characteristic of common variable immunodeficiency.

Hypogammaglobulinemia is a disorder that is caused by a lack of B-lymphocytes and a resulting low level of immunglobulins (antibodies) in the blood. Immunoglobulins play a dual role in the immune response by recognizing foreign antigens and triggering a biological response that culminates in the elimination of the antigen.

Antibody deficiency is associated with recurrent infections with specific types of bacteria. In pure B-cell disorders, cellular immunity generally is intact and the frequency of viral, fungal, and mycobacterial (e.g. tuberculosis) infections is not increased. There are five major types of immunoglobulins: immunoglobulin G, immunoglobulin M (IgM), immunoglobulin A (IgA), immunoglobulin D (IgD), and immunoglobulin E (IgE).

The most common congenital abnormalities of B lymphocyte production include:

  • Hypogammaglobulinemia (Common Variable Immunodeficiency)
  • Ig A Deficiency
  • X-linked Agammaglobulinemia (Bruton Disease)
  • Transient hypogammaglobulinemia of infancy

Patients with B-cell deficiencies begin having bacterial infections when aged seven to nine months, when the placental antibodies fall to undetectable levels. Symptoms are related to the severity of the immunodeficency, which may include the following:

  • Respiratory infections
    • Paranasal sinusitis, bronchitis, chronic cough
    • Chronic otitis media (ear infection)
    • Development into more serious respiratory conditions
      • Chronic lung disease
      • Pneumonia
      • Chronic bronchiectasis
      • Interstitial emphysema
  • Gastrointestinal disorders
  • Chronic diarrhea, weight loss and malabsorption of food secondary to Giardia Lamblia infection
  • Autoimmune disease – Rheumatoid arthritis, vitiligo, hemolytic anemia,
    thrombocytopenia, and neutropenia
  • Malignancy – risk of certain malignancies is high
  • Growth retardation in those with early-onset recurrent infections

Primary B-cell defects and most combined immune deficiencies, are generally caused by genetic defects. There is, however, a heterogeneous group of disorders known as acquired or secondary hypogammaglobulinemias that are much more difficult to define.

Supplements:

Selenium – Preliminary research reports that selenium can be beneficial in the prevention of several types of infections, including recurrence of erysipelas (bacterial skin infection associated with lymphedema) or Mycoplasma pneumonia. Further research is needed to confirm these results.

Avoid if allergic or sensitive to products containing selenium. Avoid with history of nonmelanoma skin cancer.

Sorrel – There are no well-conducted published studies that demonstrate sorrel to possess activity against viruses or bacteria that are important human pathogens. In an in vitro study, sorrel did not demonstrate activity against herpes simplex virus-1, herpes simplex virus-2, HIV, as well as Bsubtilis, E. coli, Proteus morganii, Pseudomonas aeruginosa, P. vulgaris, Serratia marcescens,or Staphylococcus aureus, which are common pathogens of bacterial infections. Further research is needed before a firm conclusion can be drawn.

Avoid sorrel if allergic to sorrel or any member of the Rumex acetosa or Polygonaceae plant families. Avoid large doses of sorrel because there have been reports of toxicity and death, possibly caused by oxalate found in sorrel. Fatal oxalic acid poisoning has been reported from sorrel soup. Be aware that many tinctures contain high levels of alcohol and should be avoided in children. These sorrel formulations may cause nausea or vomiting when taken with the prescription drug metronidazole (Flagyl®).

  • Take Vitamin D3 50,000-100,000 International Units a day for periods of 4 weeks at a time.
